Hillary Clinton Announces Health Care Agenda

Hillary Clinton Announces Agenda to Improve the Quality of Health Care for All Americans

As President, Hillary would fundamentally reform the nation’s health care system by lowering costs, improving quality, and covering all Americans. Earlier in the campaign, she announced a multi-faceted plan to lower costs and increase value in the nation’s health care system, which taken together would lower national health spending by at least $120 billion a year. Today, she announced several proposals that build on those initiatives to ensure high quality care by empowering health professionals, patients, and private and public payers to improve the financing and delivery of health care that every American receives. And next month, she will unveil her proposals to ensure universal coverage, so that every American will have quality, affordable health care.

Hillary’s agenda returns patients to the center of the health care system again by empowering and relying on the skill of those who provide care – physicians, nurses, other clinicians, and health care organizations – to improve that care continually.

Hillary’s Agenda:  Reforming Health Care by Focusing on Quality

A reformed high-quality healthcare system must include a strong evidence base so we know what treatments work best, a quality measurement infrastructure based on this evidence to assess the quality of care patients receive from their health care providers, the integration of those quality measures into the delivery of care through health information technology, reimbursement incentives and accountability, and a commitment to placing patients at the center of the system, through increased transparency and greater access to information.  To achieve this, Hillary will:

Empowering Providers: 

1.  Empower Physicians to Improve Quality Through Physician-Driven Certification Programs:   In our health care system today, there is tremendous variation in the cost and quality of care from state to state, and even from hospital to hospital in the same town.  For example, in Wilkes-Barre, Pennsylvania spine surgery rates are 1.99 per 1,000 Medicare patients, while in Lancaster, they are 5.44 per 1,000.  Physician specialty boards, such as the American Board of Internal Medicine, have established Maintenance of Certificate (MOC) programs to promote lifelong learning and help doctors stay up to date on the latest scientific advances and procedures.  Doctors who scored higher on MOC examinations had better outcomes in treating patients with diabetes, and were more likely to ensure that their patients received mammograms. Heart attack patients treated by board certified doctors were 15% less likely to die than those treated by non-certified doctors.  MOC programs help ensure that the right care is given to patients at the right time, improving quality, while reducing costs.  Hillary’s proposal would federally recognize these initiatives and create financial incentives for participating in them.

2.  Recognize Independent Private-Public Quality Trust and Improve Quality Measures:  As President, Hillary would direct the Secretary of HHS to invest $125 million in federal funding to recognize a private-public consensus-based organization, such as the National Quality Forum, to (a) certify for enhanced reimbursement physician, hospital, and nursing home MOC programs, (b) identify gaps in existing national quality measures, (c) set national priorities for the development and testing of new quality measures, (d) fund organizations with expertise to develop additional measures that advance national priorities, and (e) endorse quality measures for use in MOC programs and more broadly by physicians, hospitals, nursing homes, and other healthcare providers.  And through the Best Practices Institute announced by Hillary earlier this year, disseminate the highest quality, most effective protocols and treatments to ensure they are used everywhere. This proposal will help hold healthcare providers to the highest standards and ensure they have the best information to inform their work with patients. 

3.  Emphasize Quality in Health Care Workforce, with Focus on Nurses:  Provide federal funding to address nursing and nurse faculty shortages and nurse retention issues by establishing innovative training and mentoring programs.  A severe nursing and nurse faculty shortage has led to understaffing and nurse-to-patient ratios that jeopardize patient safety in hospitals and limit the availability of care outside of hospitals.  America is not training enough nurses to meet our needs yet nursing schools turned away more than 32,000 qualified applicants in 2005.  And we are not retaining those who do join the profession; currently 50 percent of new nurses leave their jobs within the first year.

Empowering Patients:

4.  Empower Patients with Information on Provider Performance: Informed consumers make decisions that promote the right kind of competition and higher quality and reflect their underlying values and preferences. However, very limited information is available to help patients understand how to choose the best providers, what treatment options are available, what works, and how to stay healthy.  Hillary would expand and improve information available to patients by making data and decision-making tools that give consumers information about their local providers readily available and understandable.  Patient-centered care should be designed to improve patient decision making as well as the quality of their health care; it should not shift health care costs onto patients.  At the Dartmouth Hitchcock Medical Center, the Center for Shared Decision Making helps breast cancer patients understand their options and make informed decisions about their treatment.  The program gets high marks from patients -  98 percent reporting that they understood their treatment choices and 96 percent reporting they understood which treatment risks and benefits mattered most to them as they made their decision.  

5.  Reduce Health Care Disparities: Racial and ethnic disparities are pervasive throughout our entire health care system.  In 2003, the Agency for Healthcare Research and Quality (AHRQ) found “that racial, ethnic and socioeconomic disparities are national problems that affect health care at all points in the process, at all sites of care, and for all medical conditions – in fact, disparities are pervasive in our health care system.”  Minorities are more likely to be diagnosed with late-stage breast cancer and colorectal cancer compared to whites, and Hispanics hospitalized for acute myocardial infarction are less likely to receive optimal care.  Infant mortality rates among African American populations are more than twice as high as those for whites.  According to the annual National Healthcare Disparities Report, released by the AHRQ, blacks and Hispanics received poorer quality care than whites on more than 70 percent of the measures. 

Empowering Purchasers and Payers:

6.  Incentivize Quality Through Increased Federal Payments: Hillary proposes providing higher payments to healthcare providers that use coordinated care delivered by teams of health professionals to treat the whole patient instead of the patients’ individual illnesses.  Most federal payments to health care providers do not differentially reward high quality providers.  Our current system of reimbursement creates barriers to collaboration among organizations and among health care professionals, leading to fragmentation in the health care system.  Doing what’s best for patients shouldn’t be bad for business.
7.  Prohibit Payment of “Never Events” in FEHBP and all Federal Programs:  The Bush Administration’s recent decision to refuse Medicare payments for preventable infections, injuries and errors--so-called “never events”--sustained during hospital stays is a positive step.  Hillary will insist that any insurance company wishing to provide coverage through the Federal Employee Healthcare Benefits Program also refuses to cover these costs.  This proposal will incentivize good care with smart reimbursement policies

These proposals to improve quality build on Hillary’s existing health care agenda: 

Institute a New “Paperless” Health Information Technology System:  Modernizing our health care system through the use of information technology will empower doctors and other healthcare providers to communicate electronically and will reduce waste and redundancy while improving safety and quality by reducing medical errors.   Overall, the RAND Corporation estimates net savings to be $77 billion per year.  An up-front and phased-out $3 billion a year investment fund would be provided to help hospitals and doctor’s offices adopt and implement HIT.   The proposal will give doctors financial incentives to adopt health IT and facilitate adoption of a system where high quality care and better patient outcomes can be rewarded.

Transform Care of Today’s Chronically Ill Population to Improve Outcomes:  The largest driver of health care costs in the nation is related to the small numbers of Americans who have multiple chronic diseases.  These diseases, such as cardiovascular disease and diabetes, account for 75 percent of our total national health expenditures and are the leading causes of death in the U.S.  A recent RAND study projected nearly $30 billion in national health expenditure savings per year after implementing disease and lifestyle management programs.  Combined with prevention and health information technology, with full participation, the U.S. health system could save $147 billion alone for better care of this vulnerable population. Senator Clinton will ensure higher quality and better coordination of care by using state-of-the-art chronic care coordination models within federally-funded programs to provide care for Americans afflicted with these costly, multi-faceted illnesses. She’ll provide incentives to participate in these chronic care management programs.

Create an Independent “Best Practices” Institute and Invest in Research for New Treatments:  Patients, providers, and payers would benefit from getting better information on what works in health care and how treatments compare to one another.  Researchers at Dartmouth have found that more care is not better care, and that inefficient care may do more harm than good.  Therefore she will create a new Best Practices Institute, which would be funded by both the private and public sectors, since its results will benefit all payers. Research will compare the effectiveness of alternative treatments such as pharmaceuticals, devices, and surgeries.  For example, information supplied by organizations such as the Drug Effectiveness Review Project (DERP) has been used in North Carolina to educate providers and improve quality of care, saving the state an estimated $80 million in 2003.  This research will facilitate the development of quality and outcomes measures for use by hospitals, physicians, nursing homes, and other healthcare providers.

(Little Rock) Calling her the Democrats’ best shot at winning Arkansas and the White House in 2008, Governor Mike Beebe today endorsed Hillary Clinton for President.

“As the first lady of Arkansas, I watched Hillary Clinton stand up for working families by helping to organize Arkansas Advocates for Children and families and leading a taskforce to improve education standards across our great state,” Beebe said. “As a Senator I have applauded her work to strengthen the Children's Health Insurance program, which increases coverage for our children in low income and working families.

“Senator Clinton has the right vision for Arkansas and the right vision for America,” Beebe continued. “And that’s why I know she is the Democrat with the best chance of winning this state in November of 2008.”
Clinton and Beebe appeared together at a campaign event downtown Little Rock, where they were joined by hundreds of enthusiastic supporters.

Clinton applauded Beebe’s first seven months in office, highlighting his success in cutting the state grocery tax in half, creating new economic development projects that have brought new jobs to the state, expanding preschool programs in Arkansas, and fighting for children’s health insurance.

This morning in Des Moines, Hillary showed that she is the Democrat with the strength and experience to beat the Republicans in 2008 and deliver the big change Americans want.

With Hillary it's not an either/or: She has 35 years of experience fighting for real change and she will use her experience to change Washington starting on day one.

Speaking personally, Hillary paid tribute to the work of all those women who helped change America - thus making her historic candidacy for change possible.

On Iraq, Hillary talked about her three-step plan to get our troops home in the smartest, safest way we can. Hillary Clinton Announces Rebuild America Plan for a 21st Century Infrastructure

Rochester, NH – In response to the tragic bridge collapse in Minnesota and the concerns it has raised around the country, Hillary Clinton today announced a series of emergency initiatives to ensure that the bridges, tunnels, and roads Americans rely on are safe. These initiatives are part of a broad plan for rebuilding America. Hillary’s plan will not only assure our safety, but create good jobs, stimulate the economy, enhance our global competitiveness, help the environment, and improve quality of life by reducing congestion.

“Something is very, very wrong when, at the dawn of the twenty-first century, in the richest country on earth, people are actually nervous about driving over bridges for fear that they’ll collapse. Or they’re worried that their levees may burst, or their highways may buckle. And let’s be clear: the degradation of our infrastructure isn’t just a serious threat to our safety – it is also a grave threat to our economy,” Clinton said. “We do not need any more warnings. We do not need any more wakeup calls. It is time to stop wringing our hands and start rolling up our sleeves. It is time for us to rebuild America.”

The bridge collapse in Minnesota underscores the necessity of a renewed national commitment to repairing and modernizing our infrastructure. The impact of Katrina on New Orleans was also a dramatic demonstration of the economic, environmental, and social consequences of our underinvestment. Hillary believes that we should not have to wait for bridge collapses and other disasters to focus on the state of our infrastructure. America needs a comprehensive infrastructure policy for the 21st Century.

More than a quarter of our bridges are structurally deficient or functionally obsolete. Public transit systems are under-funded and insufficiently integrated into local land use policy. Highway expansion has not kept pace with traffic growth. Seaports are too capacity constrained and technologically inefficient for a nation deriving a quarter of its gross domestic product from international trade. Waterway repairs are so long overdue that half of the nation’s locks are now functionally obsolete. Freight rail capacity constraints are slowing down commerce. And inter-city passenger rail is underdeveloped, meaning increased highway congestion and lost opportunities to create new jobs.

Hillary Clinton is announcing her Rebuild America Plan to address these challenges:

Emergency Initiatives Addressing Safety:

Establish a $10 billion “Emergency Repair Fund” to address the backlog of critical infrastructure repairs. There is a significant infrastructure funding gap, and it applies to everything from roads and bridges, to waterways and seaports. The under investment harms our economy and compromises our safety. There is an urgent need to repair and upgrade more than 60,000 bridges the Federal Highway Administration classifies as “structurally deficient.” Hillary will invest $10 billion over 10 years to finance the redesign and reconstruction of these structures. She will end the “repair-only-when-it-fails” policy that defers critical repairs because funding is unavailable.

Provide $250 million in “Emergency Assessment Grants” to the states to conduct immediate safety reviews of their high-priority, high-risk infrastructure assets. The Federal Highway Administration (FHA) has issued an advisory that urges state transportation departments to inspect bridges similar to the one that collapsed in Minnesota. Hillary Clinton proposes $250 million of “Emergency Assessment Grants” to help states conduct emergency infrastructure reviews of bridges and other critical infrastructure to ensure their immediate safety. These grants should be available with a priority-procurement status so they can be put to use as quickly as possible. If more funding is needed, Hillary will allow states and local governments the flexibility of using existing federal infrastructure funding.

Form a commission to carry out a comprehensive assessment of our engineering review standards so that we better prioritize needed repairs on bridges and roads. Recent events highlight the need to assess the integrity of our infrastructure and of prioritizing repairs to protect the safety of users. These events also suggest that we need to review our safety certification process and standards. The bridge collapse is only the most recent example of safety lapses. Last July, tons of concrete fell from the ceiling of Boston’s new tunnel and crushed a woman in her car. Hillary Clinton will establish a commission, under the auspices of the National Academy of Engineering, and the National Institute of Standards and Technology, to review the safety certification process and standards. The commission will also address the prioritization of needed repairs to roads and bridges, and make recommendations on how best to inspect, monitor, and maintain infrastructure on an ongoing basis.

Modernizing Our Infrastructure:

Public Transit

Increase federal funding for public transit by $1.5 billion per year. Increased public transit usage is arguably the best strategy for ameliorating the energy and environmental costs of transportation. As energy costs rise, more people will rely on public transportation. Today, only 5% of Americans commute by public transit, but doubling that figure could reduce carbon dioxide emissions by 25%. Public transit is also critically important to people who live in urban areas and rely on buses and trains for travel to work and school. Moreover, as the population ages, an increasing number of people will need public transit as their ability to drive diminishes. Hillary will increase federal investment in public transit by $1.5 billion per year to ensure needed capacity expansions and service level improvements.

Link federal public transit funds to local land use policies that encourage residential developments that maximize public transit usage. Over the next 25 years, a large percentage of the buildings we live, work, and shop in will be rebuilt or newly built. This presents a significant opportunity for the federal government to encourage sensible residential and commercial development that are linked to, and encourage, public transit usage. Local areas seeking large federal investments in public transit are already required to have land-use plans and policies that make investing in a high-density transit system worthwhile. Today, these requirements are focused mainly on commercial developments and not enough on residential considerations. Hillary will encourage the sort of dense residential concentrations needed to support public transit systems by better linking public transit funding with residential land-use policies. This will help to discourage sprawl and fight congestion.

Intercity Passenger Rail

Invest an additional $1 billion in intercity passenger rail systems. In the 21st Century, intercity passenger rail should be a viewed as a critical component of the nation’s transportation system. It is an environmentally efficient alternative to highway driving and short flights; it relieves congestion on roads and airports; reduces the emission of automotive pollutants; and it stimulates economic growth by linking metropolitan areas. States have been left to pursue intercity rail projects with only modest federal support. Hillary believes that greater federal involvement is needed to maximize the potential of this transportation mode. She will increase federal investment in intercity passenger rail by $1 billion over 5 years in order to help finance capital projects. These investments are in addition to those made in Amtrak.

Seaports

Partner with state and local governments and the private sector to create a national policy to guide the modernization of ports. Given the importance of foreign trade to our economy (26% of GDP), ports should be a priority for the federal government. The movement of freight by sea is expected to triple by 2020. Today, a large number of ports are in substandard condition. Many are cramped; navigation channels are narrow; harbors are too shallow for larger ships; access routes are congested; and investments in waterway managements have been low. Hillary will work with state and local governments and the private sector to devise a coherent and comprehensive national policy to expand our port capacity and our port-to-nation transportation connections. She will also work with industry to improve port productivity through such things as the deployment of new technology and by addressing congestion at the major terminals.

Addressing Congestion and Traffic Growth:

Increase the budget for the Department of Transportation’s congestion reduction programs by approximately 50% to $600 million annually. Federal support for innovative state and local congestion-reduction initiatives flows principally through two programs: Urban Partnerships and the Value Pricing Pilot Program. The combined budget for these two programs is approximately $400 million. Hillary will increase the budgets by 50% to $600 million annually to enable the programs to provide greater support to a larger number of cities and states devising innovative solutions to congestion.

Make “technology solutions” to congestion a priority for the Department of Transportation. Congestion is adding about 37% to the length of the average trip during peak hours, a three-fold increase from 1983. Road construction alone cannot be the solution to congestion because it is costly and environmentally impractical. Also, capacity additions alone will not eliminate congestion problems because traffic disruptions from things like weather, breakdowns, and road work account for one third to one half of all delays. Many traffic disruptions can be addressed by using advanced technologies to collect and relay real-time information on road and travel conditions. Also sophisticated traffic lanes management and more rapid incident clearance are shown to significantly reduce congestion. Electronic toll systems reduce backup at toll booths. In sum, the roadways of the future must integrate smart technology. Hillary will make developing and implementing technology-based congestion-reduction strategies a priority for the Department of Transportation.

Promote telecommuting by encouraging its use at federal agencies, and by committing up to $50 million per year to support state and local initiatives. Work travel imposes a greater service requirement on the transportation system than any other travel purpose. Telecommuting, the use of information technology to replace work-related travel, can ameliorate congestion and air pollution by reducing rush-hour car trips. Telecommuting can also benefit employers by reducing office space needs and increasing worker productivity. Hillary Clinton will promote telecommuting by requiring the federal agencies to set specific telecommuting goals for their workforces, and she will require that each agency task a senior manager with oversight of its telecommuting policies. Hillary will also invest up to $50 million annually in state and local telecommuting initiatives.

Broadband:

Support initiatives to establish leadership in broadband. Under the Bush administration, the country that invented the Internet has slipped to 25th in the global rankings for broadband deployment. In order to accelerate the deployment of sophisticated networks, Hillary Clinton will provide tax incentives to encourage broadband deployment in underserved areas. She will also provide financial support for state and local broadband initiatives.

In addition to the policies announced today, as Senator, Hillary has cosponsored the National Infrastructure Bank Act of 2007. The Act establishes a federally-backed independent entity that will evaluate and finance large infrastructure projects that are of regional or national significance. The bank will finance projects through the issuance of bonds backed by the full faith and credit of the U.S. government, direct subsidies, and loan guarantees.

Paul Williams Endorses Senator Clinton

Paul Williams Endorses Senator Clinton Eagle Academy Co-Founder and past president of the New York Chapter of 100 Black Men, Inc.

Paul T. Williams, Jr. today announced his endorsement and enthusiastic support for Senator Hillary Clinton’s candidacy for President of the United States. Williams will chair the urban agenda working group for the campaign.

"Hillary Clinton has demonstrated outstanding leadership abilities,” said Williams, former president of the New York Chapter of 100 Black Men and co-founder of the Eagle Academy for Young Men. “She has a record of years of commitment to issues I care about and I have learned over time that we share the philosophy that people really count! She understands that many of the problems we face can be solved by supporting people of good will, who are trying to make a difference in society,” he added.

“I am honored to receive the endorsement of my friend Paul Williams,” said Clinton. “I worked with Paul when he served as president of the New York Chapter of 100 Black Men and worked with them to establish the Eagle Academy for Young Men in the South Bronx,” she added. The Eagle Academy, a high school for minority men whose principles are built on a set of mentoring relationships between students and successful minority men, is being replicated across the nation.

“Through the urban agenda working group, I hope not only to identify great initiatives to replicate regionally or nationally, such as the Eagle Academy, but to also stimulate creative approaches to support these initiatives financially,” Williams said.

Hillary Surges In First Post-Debate Poll In SC

FIRST POST-DEBATE POLL IN SC: HILLARY SURGES Clinton Gains 18 Points Since May

In the first poll taken after Monday’s Democratic debate in South Carolina, Hillary Clinton has surged to a 15-point lead among Palmetto state voters in the latest SC Insider Advantage/Majority Opinion poll taken Tuesday.

Clinton gained 18 points since June, when she trailed by 3 points. Since April, Clinton has gained 29 points in the poll, turning a 14-point deficit into a 15-point lead.

Today’s poll shows:

Clinton: 43%
Obama: 28%
Edwards: 13%
Richardson: 5%
Biden: 4%
Gravel: 1%
Others: 1%
Undecided: 5%
